For the Primary Care Physician:
Every patient that has an upper level diagnosis is notified of the Nurse Navigator option - completely free of charge. This added support ensures that each patient understands the sequence of events in the screening/diagnostic process. Any barriers to testing and treatment completion are identified.
Your Nurse Navigator and the support team can assist the clinic with outreach events and distribute patient education materials for distribution in the clinical office.
Your Nurse Navigator can provide assistance, resources and support in order to diminish and eliminate barriers to follow-up, not only for the insured but also the uninsured and underinsured.
Your Nurse Navigator Team ensures a seamless continuity of care to your patient, newly diagnosed with cancer. For example, a breast cancer patient will need a consult with a breast surgeon, medical oncologist and radiation oncologist. The Nurse Navigator Team can assist you and your office by expediting the scheduling of appointments for ancillary testing and treatment planning.
Once the patient is referred to the Nurse Navigator, the patient will be contacted to schedule a "face-to-face" meeting with the Nurse Navigator where an evaluation of physical, emotional, and educational needs are discussed. The patient will be given an organizer with contact information for their personal providers. The PCP will receive frequent communication from the Nurse Navigator Team regarding the patient’s status and well-being.
The Nurse Navigator specializes in breast cancer diagnosis and treatment. Your Nurse Navigator can help the PCP to determine risk factors for the patients.
